The University is self insured and does not purchase insurance through Blue Cross/Blue Shield; rather the insurance company administers the health and dental insurance plans for the University. Under these indemnity plans, an employee needs to meet an annual deductible, and needs to co-pay a percentage of covered changes up to an annual stop-loss amount. A claim form must be filed, although preferred medical providers (PPOs) file claims for employees directly with Blue Cross/Blue Shield. Blue Cross/blue Shield coverage is effective worldwide. A policy book, provider book, and identification card will be sent to your home address within 4-6 weeks of the effective date of your insurance coverage. Blue Cross/Blue Shield health insurance offers three options: High, Basic, and Low; however, the medical coverage is the same for all the options. The differences are the costs to employees in terms of premiums, deductibles, co-pays , and annual stop-losses. All covered employees receive savings, no matter what option has been selected, if a PPO provider is used. A list of preferred providers can be found at.http://www.nebraska.edu/hr/hr_benefits.shtml You may also call Blue Cross/Blue Shield at 888-368-2227 for a current list of PPO providers. |
