When your employment with the University ends, your eligibility for employee benefits will change, affecting coverage and options available to regular employees who participate in the benefits program.

When Insurance Coverage Ends

You will remain covered under active benefit plans through the end of the month in which your separation date occurs.

Continuing Coverage Through COBRA

Coverage may be continued for up to 18 months utilizing COBRA insurance if application is made within 60 days from the date you lose coverage. 

  • COBRA insurance is a continuation of your medical, dental, and/or vision insurance if you are currently enrolled at your date of separation.
  • A COBRA notice will be sent to your permanent address from Health Equity, Inc after your employee benefits have ended.
  • If you enroll, COBRA will be effective retroactively to the first day following loss of coverage so there is no gap. 
  • The health risk assessment enhancements are not included in COBRA.

Additional Insurance & Plans

Group Life Insurance

Your eligibility for insurance under the group life policy issued by Assurity Life Insurance will be terminated. Contact the Group Life Department of Assurity Life Insurance at 800-869-0355 ext.3755 and reference Group #G00406 for rate and coverage information if you wish to convert to an individual policy.

Flexible Spending Accounts

Expenses incurred while you are benefits eligible until the end of the month you leave university employment can be submitted for reimbursement. You will have 90 days from the date you lose coverage to submit claims to Health Equity/Wageworks for reimbursement. Contributions after that period will be forfeited.

Retirement Savings Plans

Your retirement plan accumulations may be accessed after your last day of employment from the university and after your last paycheck has been deposited. You have several options regarding your retirement funds including keeping the funds in the account, rolling them over to a private IRA, or processing a cash withdrawal. 

  1. To begin the process of accessing your retirement funds, you will need to sign our university impact release form which can be done via DocuSign.
  2. Upon receipt of your signed impact release form and after your last paycheck has been deposited, we will notify the vendor of your date of separation from the university. 
  3. Your next step is to contact the investment company to request a distribution of your retirement funds.

Retiree Health Insurance

If you are retiring from the university, you are eligible to continue the medical and/or dental plans as a retiree. Please review the Step-by-Step for Retiring Employees which includes the retiree enrollment and authorization agreement forms or come to our office at 408 ADMS. You have up to 31 days after your active employee benefits end to enroll in retiree insurance.

BE AWARE

If you are Medicare eligible

The retiree medical plan is not considered credible coverage by Medicare and you may be assessed a penalty if you later decide to enroll in Medicare.
